    12 hours to notice worker died - The Australian
    Mr Whitehead said the union had 'grave concerns' about safety at the site.

    'This worker was missing for more than 12 hours without the alarm being raised,' he said. 'Security failed to notice heavy machinery still running unattended and the company failed to provide fall protection around the settlement pond to prevent this, as required by the Occupational Health and Safety Act.'
