Unsafe Placement of Trailers Among Six Safety Issues Identified by CSB in BP Refinery Query - Occupational Hazards The unsafe placement of mobile trailers - which may have contributed to the deaths of 15 workers in a March 23 BP refinery blast - is one of six safety issues identified in a preliminary report to be presented tonight by the U.S. Chemical Safety and Hazard Investigation Board (CSB) at a public meeting in Texas City, Texas. The six safety issues identified by CSB in its investigation of the March 23 incident, which killed 15 workers and injured 170 others, point to an incident that could have been prevented or at least minimized.
