    Woman dies in workplace accident - By BRIAN HUBER, Waukesha Freeman, WI
    The incident occurred at the Boelter Co. on Ridgeview Parkway at about 9:56 p.m. Tuesday when the woman reportedly fell from the top of an order picker, a machine similar to a forklift. She fell 12 feet to 17 feet, landing on the concrete floor below, Funkhouser said.

    Dingley Press to pay OSHA fines - By Michael Reagan, Brunswick Times Record, ME
    Kayla Cox, 21, of Dixfield, died in a printing press accident at Dingley Press. She appeared to have been caught between a paper roll and the frame of the machine, said Karen Billups, OSHA assistant area director in Augusta. The press was running at the time of the incident and Cox died instantly.

    The company was assessed two fines of $6,300 each. One was for not having a written operating procedure for the press and the other was for a lack of machine guarding.
