Worker dies after falling from scaffold - The Arizona Republic
A 29-year-old Phoenix worker died Tuesday, less than six hours after falling 20 feet from a scaffold in a house under construction in north Scottsdale.

Beelman said a preliminary investigation indicates that Helsene stepped from a ladder to a guardrail over the opening of the elevator shaft, trying to gain access to a scaffold.'He stepped on the guardrail and the guardrail broke, and he lost his balance and fell through the opening,' Beelman said of the preliminary investigation.

After the case, Dean Baker of the HSE said: "The installation of a simple gin wheel or a hoist on the scaffold would have prevented this unfortunate accident.
