Primary Power of Canada Company fined $80,000 for health and safety violations Canada NewsWire
On September 5, 2002, a worker was attempting to unplug material caught in an "ash hopper" (equipment which contained hot ash material produced by wood fuel being burned in boilers) when hot ash, dust and fire from the hopper forced the worker to jump off a railing and then off a platform. The worker fell more than 3.4 metres (11 feet) to the ground below and broke both ankles. The incident occurred after the worker opened a manhole door on the ash hopper releasing the hot ash. At the time the worker was standing on a railing which was located on a platform. The worker had climbed up on the railing in order to reach the hopper's manhole door and stoke the material inside the hopper. The worker has not returned to work since the incident.
