    Oregon - Man dies a month after being exposed to substance in tank
    Smith was aware, his uncle said, that a Spencer employee, Thomas Cassell, 45, of Oregon City, died shortly after being pulled unconscious from a gasoline tank he was working on in October 1999 in The Dalles.

    The company was cited and fined by Oregon OHSA for violating federal safety violations related to Cassell's death. Spencer has been cited and fined four times since 1994 for other safety violations in Oregon.

    Ohio - Accident kills AK Steel worker
    Outside the plant's union hall, a plaque reads:
    In memory of those who have lost their lives at Middletown Works.
    It was formerly known as the nation's most dangerous steel plant. Between 1972 and 1996, 15 workers died there, resulting in hefty government fines. Since then, the 2,791-acre complex turned around its record and has won many national safety awards.

    The death comes slightly more than a year after Elliot Munoz, 21, a contract worker at the ConAgra plant, had his legs severed in a large meat grinder on June 23, 2002

    Pennsylvania - Oil refinery fined for fatal tank explosion
    Just hours before Boles appeared in court, more than 11/2 tons of potentially lethal hydrogen sulfide gas was released into the air at the Delaware City refinery when a compressor failed and several units were shut down, according to a report received by state environmental officials.

    Prosecutors alleged Motiva caused the death of Davis, a boilermaker who was on a catwalk above the acid tank, "as a result of the use of an ignition source in an unsafe environment." Davis, 50, of Fairless Hills, was an employee of maintenance subcontractor Washington Group International of Cleveland.

    The explosion, which investigators said was ignited by a welding spark, spilled more than one million gallons of sulfuric acid. Davis' body was never found.
