There is also a morning ritual in which department heads ask workers whether they have any safety concerns. Employees who feel they are being harassed because of their safety concerns can talk privately to outside investigators about bosses eager to take shortcuts.
Dismissal of an employee cannot happen today until a plantwide panel of managers determines whether the firing in any way stems from that worker's concerns about safety.
Annually, a contractor will do a comprehensive review of the plant's safety culture and compare it with the plant's own assessments.
Finally, the company created a computerized system of reporting conditions in the plant that might threaten safety. The system allows any employee to file a 'condition report' and then follow the outcome, which typically includes a 'corrective action' and ultimately a work order to fix the problem.
