OSHA probes fatality - By nita birmingham, Charleston Post Courier, SC
GOOSE CREEK - A freak accident in which a 64-year-old North Charleston man died after tumbling inside an empty concrete mixer Monday might have been preventable, officials said Tuesday.
Though an investigation by the state Occupational Safety and Health Administration office is still in its infancy, initial reports indicate that the electrical power to the mixer might not have been shut off and secured.------------------------------------------- posted 5:03 AM :: reference link ::
