    Frame fall blamed on builder: 'We conclude that Q is exclusively responsible for the incident,' the Chadwick report said.

    It says a rushed and tired Q crew - one key executive was awakened from a nap shortly before the event - overlooked the hazards created by failing to bolt the 820-pound frame to the stage, as shown in its own design.
