The crash happened as a work crew was taking apart the scaffolding that they were standing on in an alley beside an Embassy Suites parking garage, investigators and fire officials said.
Co-workers of the injured men were waiting in a convenience store parking lot behind the building to see when — or whether — they would be allowed back to work after the early morning accident. Work had stopped during the intense rescue and investigation that followed.
Something went horribly wrong just after 7 a.m. as the crew handed pipes and large pieces of wood through a sort of human chain down about six stories.
The sounds that followed were horrifying.

The U.S. Department of Labor's Occupational Safety and Health Administration requires employers to provide training by a competent person for every worker who will be involved in putting up or taking down scaffolding. According to OSHA's Web site, that person must be able to identify potential hazards and have the authority to promptly correct them.
