The construction company involved in a Walnut Creek pipeline explosion that killed five men this month has now had eight job site deaths since October 1998 and has received 32 citations for alleged safety violations in the past decade, a record far more troubled than its major Bay Area competitors.
A Chronicle review of Cal/OSHA records and other documents shows that Mountain Cascade Inc. of Livermore has had 10 serious accidents since 1995, including the death of a crane operator whose rig toppled. In another incident, an employee nearly died after being overcome by sewer fumes while working on a job - a case that prompted regulators to issue nine citations.
