Collapse of beams may slow I-275 job By JEAN HELLER, sptimes.com
ST. PETERSBURG - Seven concrete beams weighing 87 tons each fell like giant dominoes Thursday from the supports of a ramp under construction over Interstate 275.

They shattered on an unoccupied section of roadway below. No one was injured.

Rte. 3 contractor faces $371G in fines By TOM SPOTH, Lowell Sun
OSHA alleges that employees at the Chelmsford work sites were exposed to cave-in hazards while working in unprotected excavations that lacked safe means of escape. At both sites, supervisors with the knowledge to spot cave-in hazards and the authority to correct them failed to do so. In addition, workers at Route 110 were exposed to falls of up to 28 feet from an unguarded walkway, while workers at Parkhurst Road faced crushing hazards from a crane that was set on unstable ground and had not been inspected for defects.
